Second dowload the appropriate intake form below.* Make sure that you completely fill out the intake form and sign where necessary.
Counseling Intakes Adult Intake Form
Counseling Intakes Youth Intake Form
(age 12yrs - 18yrs)
Counseling Intakes Child Intake Form
(age 5yrs - 11yrs)

Then bring the signed intakes with you to your first counseling appointment.
